Immaculate Home Health's Disease Management Program is focused on qualified patients suffering from:

  • Congestive Heart Failure
  • Chronic Obstructive Pulmonary Disease
  • Hypertension
  • Diabetes

Enrollment in the Disease Management Program entails:

Monitoring  in patient's homes: Blood pressure, pulse ,weight , pulse oxymetry, blood sugar and  PT/INR

Monitoring of  data: Nurses are assigned to monitor each patient's vital signs on a weekly basis. Readings that go beyond the established parameters are coordinated with physicians, patients, clinicians and all staff involved in the patient's care.

Following established clinical pathways: For each disease managed under the program, all clinicians follow a set of activities under the physician approved Plan of Care.

Telephonic encounters: In addition to the Nurses' regular visits, patients are called by telephone, on regular pre-scheduled intervals, no less than twice each week. The Nurse assesses the patient's current condition, monitor compliance, and conduct teaching.

Home exercise program: After a thorough evaluation, a Home Exercise Program is established by trained and certified clinicians as an essential component in managing the disease. Compliance is monitored regularly in the follow-up calls.
